Hepatitis C represents a significant public health challenge, and the World Health Organization (WHO) has set the goal of its elimination by 2030. The first step toward achieving this objective is assessing public awareness of the disease. The present study aimed to evaluate the knowledge of individuals attending authorized pre-marriage testing centers in Tehran (October to February 2022) regarding transmission, treatment, and the necessity of Hepatitis C screening. This cross-sectional study included 2,534 participants aged 14 to 84 years. Findings revealed that 30.3% of participants had never heard of Hepatitis C, over 70% were unaware of the organ primarily affected, and approximately 80% did not know that the disease is treatable. The mean overall knowledge score regarding transmission, treatment, and the necessity of screening was estimated at 38.77%. Participants with longer study hours had significantly higher knowledge scores. These results underscore the urgent need for educational interventions, public awareness programs, and the institutional strengthening of Hepatitis C screening initiatives.
