Introduction: Acidosis-Alkalosis imbalance is seen in many serious viral diseases and Covid-19 is one of these terrible diseases. Monitoring the acid-base balance of patients by ABG is a necessary examination in these patients. The purpose of this study is to investigate arterial blood gases and acid status and their relationship with the outcome of patients hospitalized in ICU. Method: The present study was an analytical retrospective study of the files of patients admitted to the special care unit of the COVID-19 hospital in Jahrom city in the years 2021 and 2022. Arterial gas analysis of patients was collected during 10 days of stay in ICU and analyzed based on demographic characteristics using ANOVA with repeated measures. Results: 296 patients with COVID-19 hospitalized in ICU with their average age of 64.37 ± 17.41 years were examined. 267 people (90.2%) had died and only 29 patients (9.8%) had been discharged from the hospital. The level of pH in deceased patients was lower than that of discharged patients, and in general, their trend was increasing over time. The amount of Pco2 in deceased patients was higher than discharged patients and in general their trend was decreasing over time. The level of LDH in deceased patients was higher than discharged patients and in general their trend increased over time. But by adjusting the effect of age, gender and duration of hospitalization, it showed that the effect of arterial gases, Acidosis- Alkalosis in time and the effect of time in the group did not show a significant difference and on the outcome of the patients with COVID-19. Admitted to the intensive care unit has not been affected. Conclusion: Our study showed that the PH level of deceased people was lower than that of surviving patients, and the level of LDH and PCO2 was higher, but it had no effect on the mortality of patients. More research is needed to fully understand the relationship between pH levels and the outcomes of COVID-19
