Comparing the Effects of Spiritual Therapy and Orem’s Self-Care Model-Based Education on Lifestyle, Anxiety, Depression, and Chronic Pain in Retired Military Elderly

Given the existing research gap and the limited attention to retirees, particularly those from the armed forces, this study aimed to compare the effects of spiritual therapy and Orem’s self-care model–based training on lifestyle, anxiety, depression, and chronic pain among retired elderly military personnel. This quasi-experimental study was conducted in 2020 on retired elderly individuals from the Islamic Revolutionary Guard Corps in Masjed-Soleyman, with approval from Baqiyatallah University of Medical Sciences. In the Orem self-care intervention group, participants received standardized educational content following a needs assessment, while the spiritual therapy group received the Richards and Bergin spiritual therapy protocol. Data were collected using questionnaires on lifestyle, chronic pain, depression, and anxiety. Data analysis was performed using SPSS software with independent t-tests, paired t-tests, and chi-square tests.
No statistically significant differences were observed between the two groups in terms of demographic variables (p > 0.05). The findings indicated that spiritual therapy had a statistically significant positive effect on depression (p < 0.001) and anxiety (p < 0.001) compared to Orem’s self-care model. However, the Orem self-care intervention did not show statistically significant differences compared to spiritual therapy in terms of lifestyle (p = 0.10) and chronic pain (p = 0.80), although clinically it had a greater impact in the Orem group. Both intervention models can be utilized to improve lifestyle and reduce chronic pain, anxiety, and depression among retired elderly individuals, particularly those from the military.
